SIMS at Berkeley, 1997
APPLICATION INSTRUCTIONS
A completed application consists of 5 items:
- Application Form. Please complete this form and remember to sign
it.>
- Transcript. Include a transcript showing courses and grades from institutions you are
currently attending. Unofficial copies of transcripts are acceptable.>
- Record of Mathematics Courses. Please fill out this form.
Use additional sheets of paper if necessary.>
- Statement of Interest. On a separate sheet of paper, briefly and candidly tell us about
your interest in the mathematical sciences, plans for future study and work, and why you would like to
participate in our program. Please limit your total statement to 500 words.>
- Faculty Recommendations: Two letters of recommendation,
preferably from professors in the
mathematical sciences, are required. You should fill out the top part of each Faculty Reference form,
and give your faculty respondents a description of the program. Please have your faculty respondents
complete the Faculty Reference Forms, and have them send the forms directly to the program, postmarked
no later than February 14, 1997.
Note: If you make photocopies of the Faculty Reference form, be sure to
copy both sides.
